Contract for bereaved carers support in Sandwell

Overview

Sandwell Council would like to hear from people whose caring role has ended, due to the person they cared for passing away.

We are looking to develop a support service for bereaved carers, and your feedback is vital in helping us shape this. This survey is anonymous and should take no more than 15 minutes to complete.

All information will be processed in accordance with UK GDPR and Data Protection Act 2018. Personal information will be held solely for the purposes of the consultation and to analyse the responses.
